SUMMARY

The Housing Navigator - Justice-Involved Program plays a critical role in assisting justice-involved individuals (JI-R) in securing stable housing as they transition from incarceration back into the community. As part of the RENEW (Reentry and Rehabilitation Services; Education and Wellness; Nutrition Programs; Emotional Support; and Wellness Activities) project at UMMA Health, the Housing Navigator works closely with clients to assess their housing needs, identify appropriate housing opportunities, and connect them to resources that support long-term housing stability. This role is essential in reducing recidivism and promoting overall wellness.

ESSENTIAL DUTIES

To perform effectively in this position, the incumbent must have: Housing Support and Navigation: Assist justice-involved individuals in locating, securing, and maintaining stable housing. Conduct housing needs assessments and develop individualized housing plans. Connect clients with emergency shelter, transitional housing, supportive housing, and permanent housing options. Support clients in completing rental applications, gathering necessary documentation, and navigating the housing search process. Advocate on behalf of clients with landlords, housing authorities, and service providers. Case Management and Resource Coordination: Work closely with case managers, peer navigators, and other service providers to ensure a coordinated approach to client care. Assist clients in accessing financial assistance programs, including rental subsidies, security deposit assistance, and housing vouchers. Provide education on tenant rights and responsibilities, budgeting, and lease compliance. Address barriers to housing, including past evictions, criminal history, and lack of income. Conduct follow-up visits to ensure clients maintain stable housing and address any emerging challenges. Community Engagement and Partnerships: Develop and maintain relationships with landlords, property managers, and housing agencies to expand housing options for justice-involved individuals. Collaborate with local shelters, transitional housing programs, and supportive housing organizations to enhance available resources. Participate in community events, housing fairs, and networking opportunities to strengthen housing partnerships. Advocate for policies and programs that increase access to housing for individuals with justice-involvement. Program Development and Evaluation: Provide feedback to the Justice-Involved Program Manager on program effectiveness and areas for improvement. Assist in tracking client progress, maintaining accurate records, and collecting data for grant reporting. Participate in team meetings, training sessions, and professional development opportunities. Program Coordination and Compliance: Attend HRSA calls and work with program consultants at the direction of the Justice-Involved Program Manager. Ensure program activities align with HRSA guidelines and grant requirements. Support documentation and reporting processes required for compliance with federal and state funding sources.

ENVIRONMENT

The environment for this position is a medical clinic that is clean and comfortable. It may include some minor annoyances such as noise, odors, drafts, etc. The incumbent is in a non-confined office-type setting in which he or she is free to move about at will.

EDUCATION, TRAINING AND EXPERIENCE

QUALIFICATIONS/ POSITION REQUIREMENTS

Regulatory Requirements: • Successful completion and acceptable results of post offer physical examination, drug screening, and criminal background check. • Recommended Healthcare Personnel Requirements for vaccinations and preventative testing: Hep B, Influenza, MMR, TDAP, TB, Covid-19 and Booster. Education and Experience: High school diploma or equivalent required; associate or bachelor's degree in social work, human services, or a related field preferred. Experience in housing navigation, case management, or reentry services preferred. Knowledge of housing resources, tenant rights, and housing subsidy programs. Experience working with individuals with justice-involvement, behavioral health needs, or housing instability. Candidates with previous lived experience of housing insecurity or justice-involvement are strongly encouraged to apply. Skills and Abilities: Strong advocacy, negotiation, and problem-solving skills. Ability to work collaboratively with a multidisciplinary team and external partners. Knowledge of community resources, housing assistance programs, and fair housing laws. Proficiency in Microsoft Office and electronic health record (EHR) systems is a plus.
