Job Description

The Nurse Practitioner (NP) acts as part of the clinical operations team and is responsible for providing direct patient care in ChenMed/Jencare medical centers, nursing homes, skilled nursing facilities (SNF) and home settings depending on the nature of the assignment or providing assessments to members in SNF and home settings. Responsibilities include geriatric assessment, medical history, physical exam, diagnosis and treatment, development of the nursing plan of care, health education, physician referrals, case management referrals, follow-up and clear documentation according to ChenMed standards for quality, service, productivity and teamwork. Participation in clinical rounds and conferences plus in-depth documentation through written progress notes and summaries is required. The NP must function independently and collaboratively with other health care professionals, consult with managers, physicians, and medical directors to ensure compliance with guidelines, and may participate in risk and quality management programs and clinical meetings. The NP adheres to departmental goals, standards of performance, regulatory compliance, quality patient care compliance, and policies and procedures. Duties include assessing acute and non-acute clinical problems, performing physical assessments and patient histories, developing and implementing patient management plans, patient advocacy, writing orders for admissions, transfers, discharges, ordering and interpreting lab and diagnostic studies, prescribing medications and treatments, referring patients for consultations, documenting progress notes, performing invasive procedures upon competency, participating in patient care rounds, collaborating with multidisciplinary teams, recognizing urgent situations requiring physician attention, and using advanced communication skills to solve problems and improve patient service. Additional responsibilities include collaborating to improve healthcare delivery systems, participating in professional development activities, arranging SNF discharges, working with care teams to transition patients to lower levels of care, facilitating patient/family conferences, enhancing collaborative relationships for informed decision-making on care goals, prescribing medications per state practice, and other duties as assigned. The position may require 50-75% local travel. Education and experience criteria include ARNP or similar advanced nursing degree, current Nurse Practitioner certification in the state of practice, preferred board certification by AANP or ANCC, possible DEA number for controlled substances, minimum 1 year clinical experience preferred, and Basic Life Support certification within 90 days of employment.
